It’s easy to become a member of Affinity Medical Group. Follow the easy steps below:

1. Select a physician.
   

   Use our online search to choose from our list of Primary Care Physicians in your area.

2. Notify your health plan.
   

   Call your health plan. The health plan’s contact information can be found on your insurance identification card. When you phone, inform the health plan representative that you want to change your primary care provider.

3. Make an appointment with your new physician.
   

   An identification card naming your newly-chosen Affinity provider will be sent to you in the mail.
